3. Conduct a background check, including but not limited to verification of my identity, criminal record, and prior business or credit activity.
4. Contact any references, employers, financial institutions, or other entities deemed necessary to verify accuracy of the information submitted.

  I understand that this authorization will remain valid for the duration of my application process and, if approved, may extend for the purpose of ongoing compliance and verification. I further acknowledge that:

• A non-refundable $100 application fee will be charged to cover administrative and third-party verification costs.
• Variable fee ($1-$100) may be applicable depending on city and state for Criminal and Civil Background check.
• Providing false, incomplete, or misleading information may result in denial of my application or termination of any pending approval.
• All information obtained will be used solely for business evaluation purposes and handled in accordance with applicable federal and state privacy laws.
